Patient Care Pharmacist

We are seeking a highly skilled and compassionate Patient Care Pharmacist to join our team at Nationwide Children's Hospital. As a member of our patient care team, you will work closely with prescribers, healthcare practitioners, and other members of the team to provide optimal pharmaceutical care to our patients.
Responsibilities- Provide direct patient care, including medication therapy management, medication reconciliation, and patient education.
- Collaborate with the multidisciplinary team to develop and implement individualized patient care plans.
- Participate in code blue and trauma responses as needed.
- Document patient care activities and outcomes in the medical record.
- Provide in-service training to medical, nursing, and pharmacy staff.
- Precept and/or supervise pharmacy residents, students, interns, technicians, or graduates awaiting licensure.
- Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
- Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio.
- Board certification highly preferred and recommended.
- Minimum 3 years of hospital pharmacist practice experience (pediatrics preferred) with responsibility for direct clinical patient care similar to that provided through a PGY1 Residency.
The Patient Care Pharmacist will work in a fast-paced environment, providing direct patient care and collaborating with the multidisciplinary team. The ideal candidate will be able to work effectively in a team setting, communicate effectively with patients and healthcare providers, and prioritize tasks to meet deadlines.
