We are seeking a Registered Nurse to join our team as a Clinical Care Coordinator. This role will involve providing clinical direction for all aspects of patient care in an acute inpatient setting, fostering a patient-centered approach. The successful candidate will have a minimum of 1 year of experience as a registered nurse and hold a current RN compact/multistate license recognized by the NCSBN Compact State or be licensed to practice as an RN in the state they are working.

Key Responsibilities
• Collects data and completes nursing assessments utilizing appropriate area and/or age-specific standards/policies and tools
• Develops individual plans of care based on the assessed needs of patients, collaborating with the interdisciplinary healthcare team
• Identifies realistic and measurable goals and outcomes to be attained prior to discharge
• Validates these priorities with healthcare team members and documents appropriately

Required Skills and Qualifications
• Associate degree in Nursing; Bachelor's degree in Nursing preferred
• 1 year experience as a registered nurse
• Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state they are working
