Role Overview

The Hospice CNA is responsible for delivering high-quality personal care under the guidance of a registered nurse. You will be working in the East Valley region, supporting our operations and making a meaningful difference in the lives of our patients.

Responsibilities
  • Document care provided according to organizational policy and procedures.
  • Report any deviations from the Plan of Care to the Case Manager.
  • Use Durable Medical Equipment safely and effectively.
  • Take vital signs when on Hospice Aide Plan of Care.
  • Foster a collaborative environment among personnel by understanding the functions of other individuals involved in patient care and actively participating in team and staff conferences.
  • Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence, reporting any changes or areas of concern to the Case Manager.
  • Assist with ambulation, transfer, and range of motion if documented on the Hospice Aide Plan of Care.
  • Provide respite for patients' family/caregiver when on-site, as appropriate.
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities.
  • Participate in Interdisciplinary Group (IDG) meetings, as appropriate.
Qualifications
  • Meet training requirements by the state of Arizona.
  • One year of experience as a CNA, hospice aide, or home health aide.
  • At least 18 years old.
  • Must be a licensed driver with an insured automobile in good working order.
  • The ability to communicate effectively, both verbally and in writing.
  • Ability to work as a team and independently.
