We are seeking a skilled Registered Nurse to join our team at YAI. As a key member of our healthcare team, you will provide high-quality nursing services to individuals with intellectual and developmental disabilities, mental health needs, and other related conditions.
Key Responsibilities- Provide nursing services, including assessments, care planning, and implementation of individualized health care plans.
- Collaborate with direct support staff, program leadership, and external providers to ensure comprehensive care and support.
- Collect and document vital signs, social history, and other relevant information to support assessments and care planning.
- Alert community physicians to any concerns or complaints received from individuals supported.
- Document all calls, correspondence, and encounters with individuals supported in accordance with established guidelines.
- Utilize the nursing process to assess, plan, implement, and evaluate individualized health care plans.
- Triage emergency and non-emergency phone calls from program staff, leadership, and individuals supported.
- Develop and maintain an understanding of relevant regulations, including Titles 17 and 22.
- Write, review, and update individualized nursing health care and restricted health care plans.
- Present information and respond to questions from multidisciplinary teams and external providers.
- Current active license as a Registered Nurse issued by the State of California Board of Registered Nursing.
- Current Basic Life Support (BLS) certification for Healthcare Providers.
- Two (2) years of experience performing professional nursing duties in any setting, including experience with individuals with I/DD, mental health, or related needs.
- Demonstrates sound clinical judgment in selecting methods and techniques to resolve medical concerns.
- Excellent interpersonal, verbal, and written communication skills.
- Proficiency with Microsoft Office, email management, and electronic health records.
- Ability to travel to, from, and/or between assigned programs multiple times per month.
- Ability to respond to and/or triage calls regarding medical concerns or emergencies outside of regularly scheduled working hours.
- Familiarity with California's Lanterman Act and Title 17 and 22 regulations.
- Two (2) or more years of RN experience working with individuals with I/DD, ideally in a residential setting.
