Ambulatory care pharmacists play a vital role in providing high-quality, patient-centered care in outpatient settings. As an ambulatory care pharmacist at Kaiser Permanente, you will work collaboratively with healthcare teams to deliver evidence-based, cost-effective pharmaceutical care. Your expertise will be essential in managing patient drug therapy, providing education, and preparing medications. This role requires strong communication skills, a commitment to quality improvement, and a passion for staying up-to-date on the latest pharmacological principles and medical terminology.
Essential Responsibilities:
- Collaborate with healthcare teams to develop and implement patient-centered care plans, incorporating pharmacological and non-pharmacological interventions.
- Conduct thorough medication histories, identify potential medication-related problems, and develop strategies to prevent and resolve these issues.
- Provide education to patients, families, and healthcare providers on medication use, disease management, and healthy lifestyle choices.
- Monitor and adjust drug therapy as needed, using evidence-based guidelines and clinical judgment.
- Participate in quality improvement initiatives to optimize pharmaceutical care services and promote a culture of safety and excellence.
- Stay current with advances in pharmacology, disease management, and healthcare policy, applying this knowledge to improve patient outcomes and care processes.
- Communicate effectively with patients, families, and healthcare teams, using clear and concise language to ensure understanding and adherence to care plans.
- Document patient interactions, medication therapy plans, and other relevant information in a timely and accurate manner.
- Perform other duties as required to support the delivery of high-quality, patient-centered care.
Experience
- Successful completion of a clinical pharmacy residency or equivalent experience in direct patient care delivery.
- B.S. degree in Pharmacy or Pharm.D. required.
- Pharmacist License (California) required at hire.
- Basic Life Support from American Heart Association.
- Comprehensive knowledge of current pharmacological principles, medical terminology, pathologies, disease states, and related information for a medical management plan.
- Working knowledge of aseptic technique and preparation of sterile IV products and chemotherapy agents.
- Understanding of drug utilization review and drug usage evaluation.
- Knowledge of Federal and State laws and regulations regarding the provision of pharmaceutical services.
- Excellent verbal and written communication skills.
- Ability to operate the pharmacy computer system(s) and other equipment.
- Ability to input prescription labels.
- Must obtain an NPI # within one month in position.
- Must be able to work in a Labor/Management Partnership environment.
- ACLS certification may be required in some areas.
