Job Summary:

The Registered Nurse (RN) position is responsible for providing high-quality nursing services to clients/patients in a variety of settings, including residential care facilities, behavioral health programs, substance use disorder treatment programs, crisis centers, outpatient and/or primary medical care clinics, or in the community within client/patient homes.

Key Responsibilities:

• Perform comprehensive nursing assessments per service line requirements, utilizing a high level of clinical competence.

• Evaluate the client/patient's physical condition and the need for services, ensuring timely and effective interventions.

• Complete required screenings, record the client/patient's initial medical histories and vital signs, and coordinate any required action planning based on results.

• Monitor the health status of the client/patient during services provided, identifying and addressing any emerging concerns.

• Work collaboratively with the treatment team to coordinate care based on client/patient needs, ensuring seamless transitions and optimal outcomes.

• Develop and implement individualized action plans for clients/patients based on physical and mental health needs, in conjunction with the treatment team.

• Oversee infectious disease control, client/patient hygiene, and related client & facility health issues, under the supervision of nursing leadership.

• Provide disease prevention, risk reduction, and other health education as needed, promoting healthy behaviors and lifestyles.

• Arrange medical appointments for clients/patients dealing with medical conditions that occur during treatment and manage medical emergencies, such as accidental injuries and/or drug reactions.

• Consult with a physician/advanced practice provider as necessary or advocate for medical services through managed care organizations.

• Arrange or monitor special dietary needs for medical conditions, ensuring optimal nutrition and health outcomes.

• Review medication requirements with clients/patients, educating the client and guardian (if necessary) about the benefits of taking prescribed medications as prescribed.

• Administer and oversee administration of medications via various methods according to service line requirements, monitoring medication compliance in regard to State Regulatory standards.

• Consult with the physician/advanced practice provider or pharmacy to confirm medications prescribed, ensuring accuracy and safety.

• Oversee therapeutic injection of medication (subcutaneous or intramuscular), following established protocols and guidelines.

• Monitor lab levels, including consultation with physicians/advanced practice providers, clients, and clinical staff, to ensure timely and effective interventions.

• Coordinate medication needs with pharmacies, clients/patients, and families, including the use of indigent drug programs, to ensure access to necessary medications.

• Monitor medication side-effects, including the use of standardized evaluations, to identify and address any emerging concerns.

• Monitor physician orders for treatment modifications requiring client/patient education, ensuring timely and effective interventions.

• Assist Physician/Advanced Practice Provider with client/patient exams, providing support and guidance as needed.

• Set up and draw any necessary labs and perform any special procedures as requested by the Physician/Advanced Practice Provider, ensuring accuracy and safety.

• Complete all required documentation in a timely manner as indicated in company policies and procedures, maintaining accurate and up-to-date records.

• Demonstrate interest in long-term and short-term goals and objectives of the company, contributing to a positive and productive work environment.

Requirements:

• Current Registered Nurse licensed in the state of practice

• Willingness to become licensed in additional states, other than the state currently licensed in, and to provide telehealth services to the individuals we serve.

Preferred Qualifications:

• BCLS certification required, ACLS certification preferred.

Work Environment:

This position is sedentary, with occasional walking or standing required. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
