As a Hospice RN Case Manager, you will play a vital role in providing direct nursing services to patients in hospice care while also offering essential support to their families. You will serve as the key communicator among the physician, patient/family, and hospice team, empowering caregivers and working collaboratively with a multidisciplinary team to ensure comfort, dignity, and peace for our patients.
Key Responsibilities:
- Deliver compassionate and empathetic care to hospice patients and their families.
- Maintain direct communication with the healthcare team to coordinate patient care.
- Empower and educate caregivers to enhance the quality of care.
- Document patient progress and care plans accurately.
Qualifications:
- Registered Nurse (RN) license in the state of employment.
- Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology settings preferred.
- Valid driver's license and automobile insurance.
- Current CPR certification.
