Job Title: Private Duty Nurse LPN

Aveanna Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. As a Private Duty Nurse LPN, you will be responsible for delivering high-quality patient care, documenting patient progress, and collaborating with healthcare teams to ensure optimal patient outcomes.

Job Summary:

The Licensed Practical Nurse (LPN) is a vital member of our home healthcare team, providing personalized care to patients in their homes. As a Private Duty Nurse LPN, you will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Deliver high-quality patient care, including administering medications, wound care, and vital sign monitoring.
  • Document patient progress, including medical history, treatment plans, and outcomes.
  • Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive patient care.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide education and support to patients and families on disease management, self-care techniques, and prevention strategies.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements:
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Must be able to sit and climb stairs.
  • Must have visual and hearing acuity.
  • Must have strong sense of smell and touch.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
Environment:
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
