Job Summary

We are seeking a skilled and compassionate Nursing Professional to join our team as an In-Home Care Specialist. As a Private Duty Nurse, you will provide high-quality care to clients in the comfort of their own homes.

Responsibilities
  • Develop, review, and revise individualized plans of care in collaboration with clients, families, and healthcare teams.
  • Provide nursing care in accordance with physician orders, client needs, and payer source authorization.
  • Monitor and record client reactions to treatment, changes in condition, and provide ongoing instruction and supervision as needed.
  • Participate in education programs, quality assessment activities, and staff meetings to ensure excellent patient care.
Requirements
  • Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
  • Excellent problem-solving skills, ability to multitask, and strong communication skills.
  • Positive, professional attitude and a commitment to delivering exceptional patient care.
