Job Summary

Solution Based Therapeutics is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in the prevention of substance abuse and the treatment of residents with substance abuse problems.

Key Responsibilities
  • Conduct intake interviews to gather personal and family data, including a study of individuals or family based on medical, psychological, social or environmental needs.
  • Determine admission criteria in accordance with all relative factors concerning the resident involved.
  • Attend and participate in clinical team meetings.
  • Initiate discharge planning upon admission for assigned residents, and update discharge plan weekly.
  • Develop and implement treatment plans, monitor and chart resident progress, and implement modifications to the resident's program as needed.
  • Perform drug and alcohol abuse lectures, as well as individual and group recovery counseling.
  • Assist in the observation of medication administration as needed.
  • Assist in completion of rounds and vital signs assessments as needed.
Requirements
  • Either be registered as a recovery counselor (CADC) or be in the process of obtaining registration, which can include attending a college or university to obtain recovery counselor certification.
  • If recovering from substance abuse, the candidate must have two years of continuous sobriety.
  • Some knowledge of self-help groups (AA, NA, CA), community resources, and charting resident files.
  • Current TB test results.
  • 1st aid/CPR certification is a plus.
