Job Summary:We are looking for a compassionate and detail-oriented Registered Nurse to coordinate care with primary care physicians, specialists, pharmacies, and other medical services. You will work closely with our multi-disciplinary team to develop and implement plans of care, ensuring the highest level of quality and safety for our individuals.
Key Responsibilities:- Coordinate care with primary care physicians, specialists, pharmacies, and other medical services
- Work with the multi-disciplinary team to develop and implement plans of care
- Direct managers and staff to prepare paperwork and oversee individual appointments
- Monitor and order medications to ensure safety and compliance
- Participate in hospital discharge planning, hospice intake, and VNA supports within the community
- Verify healthcare binders are kept organized and current and ensure healthcare records are entered in HCSIS and kept up to date
- Facilitate health education and teaching to direct care staff and participate in other trainings/meetings required within the department
- Periodically visit assigned residential sites to review medications and discuss issues or educational needs with staff
- Conduct medical audits to ensure compliance, safety, and well-being of all involved
- Travel to surrounding areas, including Malden, Reading, Saugus, Danvers, Stoneham, Beverly, and Peabody
- MA RN License (required)
- Knowledge of case management systems, community service, and delivery systems for DD population (required)
- Attention to detail and excellent time management skills (required)
- Strong clinical communication skills (required)
- Minimum of 1-year experience working with DD adults in long-term care or human service agency (preferred)
- Experience with staff training and development (preferred)
- Ability to verbally communicate
- Ability to read, analyze, interpret, and write documents
- Ability to respond and navigate common inquiries or complaints from regulatory agencies or family members
