Harbor Care is seeking a talented and committed Registered Nurse to join our team at the Harbor Care Health and Wellness Center. As a Registered Nurse, you will be responsible for providing skilled personal care and health-related services to our patients.
Job Summary:The Registered Nurse (RN) is a professional employee who functions under the direction, instruction, and supervision of the Nurse Manager/Program Manager designee. The RN is responsible for providing anticipatory guidance, screening, and health education to patients to promote and maintain their health.

- Initiates treatment for patients with commonly occurring health problems within the nurse specialist's scope of practice.
- Responsible for tracking and outreach of patients in the Hepatitis C program.
- Monitors patients with stabilized long-term or chronic illness and aids and supports individuals and their families.
- Plans appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and agencies.
- Instructs individual and/or families or groups about health or defined health problems.
- Participates in Quality Assurance activities.
- Participates with providers to ensure timely and effective care for patients.
- Participates in education of graduate nursing students, as assigned.
- Promotes community relations; anticipates and responds to the needs of others; provides assistance in a courteous, quick manner; treats others with care and respect maintaining privacy, confidentiality, and dignity.
- Maintains a neat, clean, and professional appearance and workspace.
- Appropriate footwear for all seasons.
- Conducts oneself in a manner that promotes safety.
- Completes all necessary paperwork related to the position and/or yourself in a timely manner.
- Other duties as required by Program Manager and/or designee.
- Associate's degree in nursing from an accredited school of nursing.
- Two (2) years of Medical/Surgical experience as a Registered Nurse.
- Current registration and licensure in the State of New Hampshire.
- Current CPR certification.
- Bachelor's degree is preferred but not required.
- Resourcefulness, flexibility, and integrity.
- Experience working in a Federally Qualified Healthcare Center organization is a plus.
- Experience in a community healthcare setting.
- Knowledge of Microsoft Office and Centricity EMR software preferred.
- Good oral, written, and reading communication skills. Have the ability to understand verbal and written instructions. Read and write documentation in patient records.
- Be aware of and adhere to HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency.
