As a LPN Home Health Care Specialist, you will play a vital role in delivering patient-centered care to individuals in the comfort of their own homes.
Responsibilities- Verify the patient's plan of care with the Registered Nurse (RN) prior to each visit.
- Collaborate with the physician and RN to ensure the patient's needs are being met, and notify them promptly of any changes in the patient's condition.
- Teach patients and caregivers self-care techniques to promote independence and improve quality of life.
- Monitor vital signs and review medications with the patient, performing a medication reconciliation at each visit.
- Perform specialized procedures ordered by the physician, such as foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation in a timely and accurate manner.
- One year of clinical LPN experience.
- Current LPN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
