We are seeking a skilled and experienced LPN to provide high-quality home health services to patients in the Andover area. As a valued member of our team, you will have the opportunity to make a positive difference in the lives of our patients and their families.
Key Responsibilities
- Conduct patient visits and ensure that in-home visit time fully meets the needs of the patient, providing personalized care and attention.
- Administer skilled nursing services to patients requiring intermittent professional services, ensuring that care is delivered in accordance with physician orders and the established plan of care (POC).
- Coordinate total patient care, including interdisciplinary communication with other healthcare providers, offices, and physicians, to ensure seamless and comprehensive care.
- Perform services in accordance with the physician's orders and the established plan of care (POC), ensuring that care is delivered in a timely and efficient manner.
- Guide and instruct patients and their families in prescribed activities that are directed toward improving independence and functionality, promoting self-care and empowerment.
Requirements
- Must be a Licensed Practical Nurse in the state of employment.
- Must have at least one year of experience as a licensed nurse.
- Must have basic demonstrated technology skills, including operation of a mobile device.
- Medicare home health or hospice experience is preferred.
- Must possess a valid state driver's license.
- Must maintain automobile liability insurance as required by law.
- Must maintain dependable transportation in good working condition.
- Must be able to safely drive an automobile in all types of weather.
