Licensed Practical Nurse II

We are seeking compassionate and professional Licensed Practical Nurses (LPNs) to join our team at the Veterans Home in Holyoke, Massachusetts. As a LPN, you will be responsible for providing patient-driven care and support to our veteran population.
Job Summary
The Licensed Practical Nurse II will provide high-quality nursing care to residents in a skilled nursing unit. This is an open and continuous posting for part-time positions on our evening and night shifts.
Responsibilities
- Provide exceptional nursing care to veteran residents by assessing health status, responding to call lights or veterans' requests for assistance, and making observations on rounds.
- Administer nursing treatments, procedures, doctor orders, and care plans; observe, identify and respond to the needs for care, including medication, equipment-assisted care and patient/family education.
- Identify, evaluate, and assess veteran health status; confer with supervising Registered Nurses (RNs) and assigned medical staff for treatment modalities.
- Recognize urgent or emergency patient care situations; seek assistance of the RN and/or medical staff and initiate appropriate emergency interventions as directed or needed.
- Collaborate with other resident services personnel to ensure that residents' individual care plans are being followed.
- Establish constructive relationships with veterans and their families to promote positive relationships and learning as established by the treatment/teaching plan.
- Document incidents with or without adverse effect to the veteran and refer to RN for follow-up and/or investigation.
- Foster the Veterans Home's mission, vision and operational policies while maintaining regulatory and department compliance with Joint Commission, and state/federal regulations.
- Maintain personal and professional growth by attending in-services, keeping current with healthcare trends and nursing practices, and other Veterans Home committees.
- Skilled in the use of computers as needed for the completion of work assignments.
Requirements
To be considered for this role, you must possess a current, active license as a practical nurse under the Massachusetts Board of Registration in Nursing. Additionally, you must have CPR certification through the American Heart Association.
Preferred Qualifications
At least one (1) year of experience in the care of geriatric patients in a healthcare setting such as long-term care is preferred. Certification in dementia, hospice, or other relevant discipline is also desirable.
