Job Summary:

We are seeking an experienced Clinical Team Leader to assist the Branch Director in ensuring the overall coordination of home health services to all clients is delivered according to acceptable standards of practice. The ideal candidate will possess a graduate degree in nursing, be licensed in the state of employment, and have a minimum of two years nursing experience and one year experience in home health.

Responsibilities:

  • Assist the Branch Director in day-to-day office and staff management related to patient care
  • Provide oversight of patient care and contribute to the overall company success of providing A Better Way To Care
  • Facilitate relationships between physicians, referral sources, patients, caregivers, and employees
  • Participate in an on-call rotation and make patient visits as needed
