Key Responsibilities- Provide skilled nursing services to patients in hospitals, long-term care facilities, and other healthcare settings.
- Administer medications, perform medical procedures, and provide wound care under the supervision of a registered nurse or medical doctor.
- Develop and implement individualized patient care plans to promote optimal health outcomes.
- Collaborate with healthcare teams to ensure seamless patient care and communication.
- Conduct thorough assessments, identify patient needs, and develop effective care plans.
- Provide education and support to patients and their families on health-related topics and disease management.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
- At least one year of experience in a healthcare setting, preferably in a long-term care or hospital environment.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to maintain confidentiality and handle sensitive patient information.
- Basic life support (BLS) certification and other required certifications.
