Aveanna Healthcare Private Duty Nurse LVN Job Description

Aveanna Healthcare is a leading national provider of home health services to medically fragile children and adults. We are seeking a compassionate and skilled Private Duty Nurse LVN to join our team.

Job Summary

We are looking for a dedicated and experienced Private Duty Nurse LVN to provide one-on-one care to our patients in their homes. As a Private Duty Nurse LVN, you will work closely with our patients and their families to develop and implement personalized care plans that meet their unique needs.

Responsibilities
  • Implement physicians' prescribed plan of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our system of record
  • Provide health education and training to family members
  • Collaborate with our Case Manager to ensure seamless care coordination
Requirements
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • 6 months prior hands-on nursing experience preferred
  • Reliable transportation
