Liberty Health is seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families facing terminal illness.
Key Responsibilities:
- Deliver high-quality patient care: Apply your knowledge of nursing theories and concepts to provide compassionate and individualized care to patients and their families.
- Coordinate patient care: Work collaboratively with the Hospice team to coordinate physical care, medication management, and other aspects of patient care.
- Communicate effectively: Communicate with patients, families, and healthcare professionals to ensure seamless care coordination and patient satisfaction.
- Participate in IDT meetings: Attend and participate in Interdisciplinary Team (IDT) meetings to discuss patient care plans and ensure comprehensive care.
- Meet reimbursement guidelines: Ensure that patient care and documentation meet reimbursement guidelines and regulatory requirements.
Requirements:
- Current RN licensure: Possess a current Registered Nurse (RN) license in the state of practice.
- Nursing experience: Have at least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience.
- Education: Hold a nursing education from an approved school of nursing.
- Continuing education: Complete 12 hours of continuing education annually to stay current with industry developments and best practices.
- CPR certification: Possess current CPR certification.
- Travel and flexibility: Be willing to travel based on agency need and work flexible hours, including weekends, evenings, and holidays.
