Job Summary:We are seeking a skilled and compassionate Wound Care Nurse to join our team. The successful candidate will be responsible for providing wound and ostomy care to patients, assessing their condition, and developing and implementing individualized care plans. You will work collaboratively with other healthcare professionals to ensure seamless care delivery and promote a culture of excellence and teamwork.
Responsibilities:- Provide wound and ostomy care to patients, including wound care assessment, implementation of wound care interventions, and consultation with physicians.
- Assess patient's condition and needs, set outcomes, and implement appropriate nursing actions to meet their physical, emotional, spiritual, social, and intellectual needs.
- Collaborate with other disciplines to ensure comprehensive care delivery and promote a culture of teamwork and excellence.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery and promote a culture of patient-centered care.
- Current valid Oregon RN license.
- Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements.
- AHA Basic Life Support for Healthcare Provider certification.
- Board Certified Wound Care or Ostomy Care Certification through ABWM or WOCNCB (preferred).
