We are seeking a skilled Patient Care Coordinator to join our team. As a Health Navigator, you will play a vital role in making healthcare easier for doctors and patients alike.

Your responsibilities will include:

  • Developing Strong Relationships: Build trusted relationships with our patients and providers to ensure seamless care coordination.
  • Patient Engagement: Meet with patients during check-in, call patients to schedule appointments, and remind them to pick up medications.
  • Care Coordination: Identify and track progress on important gaps in patient care, order procedures and tests, and maintain electronic health records.
  • Patient Education: Educate patients on their healthcare options, insurance benefits, and common medical conditions.

To succeed in this role, you must be a self-starter who is comfortable working independently, enjoy meeting new people, and bring a strong service mentality to your work. You should also be flexible, excited to tackle new challenges, and comfortable using data to inform decisions.

The estimated salary for this position is $55,000 per year, based on national averages for similar roles. We offer a comprehensive benefits package, including health, dental, and vision insurance, 401(k) matching, and paid time off.
