Key Responsibilities
  • Deliver patient-centered care according to individualized care plans and needs
  • Perform wound care treatments and assist clients with medication management
  • Teach clients and/or family members about care and support skills
  • Communicate effectively with RN Case Managers, Supervisors, and the Care Team
Requirements
  • Licensed as a Practical Nurse/LPN in the state of Minnesota
  • Valid Driver's License and reliable vehicle with current insurance
  • Demonstrated ability to work autonomously and as part of a team
  • Prior nursing experience in a TCU, Hospice, or Home Health setting
  • Experience with Electronic Medical Records (EMR) software preferred
