Key Responsibilities- Client Assessment and Care Planning: Conduct thorough assessments of clients' physical, emotional, and spiritual needs, developing individualized plans of care that address their unique needs and goals.
- Collaboration and Communication: Work closely with physicians, social workers, chaplains, and other healthcare professionals to ensure seamless care coordination and effective communication with clients and their families.
- Symptom Management and Palliative Care: Anticipate and manage clients' symptoms, providing expert care to alleviate pain, discomfort, and distress.
- Education and Support: Educate clients and their families on disease management, self-care, and end-of-life care, providing emotional support and guidance throughout the care process.
- Documentation and Reporting: Maintain accurate and timely documentation of client care, including progress notes, treatment plans, and other relevant information.
- Education: Graduate of an approved Nursing education program.
- Licensure: Current Registered Nurse licensure in the state of practice.
- Experience: Minimum of one (1) year experience as a professional acute care nurse, with home healthcare/hospice experience preferred.
- Skills: Strong communication and interpersonal skills, with a special emphasis on grief/loss management.
