Your expertise will be utilized to develop and provide clinical services in your area of specialization, conducting drug therapy monitoring, drug formulary reviews, and making pharmaceutical interventions to reduce adverse events.
Key Responsibilities:- Pharmaceutical Care: Provide pharmaceutical care in your specialty practice area, including review and evaluation of patient drug/medical history, providing drug therapy recommendations to optimize care, and providing drug information.
- Clinical Process Improvement: Plan, implement, perform, and measure clinical processes to improve treatment outcomes, minimize adverse drug events, and contribute to quality and financial outcomes.
- Staff Development and Training: Create a culture of learning and sharing that supports the development of a qualified clinical staff through coaching, motivation, and empowerment. Implement staff development on clinical processes and participate in the training of pharmacists, pharmacy residents, pharmacy students, and healthcare providers.
- Interdisciplinary Teamwork: Promote interdisciplinary teamwork by developing and implementing clinical pathways and protocols that promote appropriate medication safety and medication utilization using best practice, evidence-based recommendations.
- Education: Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy.
- Experience: Two years of experience or completion of a PGY1 residency.
- Licensure: Pharmacist licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
- Certifications: Board Certified Ambulatory Care Pharmacist (BCACP), Board Certified Cardiology Pharmacist (BCCP), Board Certified Critical Care Pharmacist (BCCCP), Board Certified Geriatric Pharmacist (BCGP), Board Certified Infectious Diseases Pharmacist (BCIDP), Board Certified Nuclear Pharmacist (BCNP), Board Certified Nutrition Support Pharmacist (BCNSP), Board Certified Oncology Pharmacist (BCOP), Board Certified Pediatric Pharmacy Specialist (BCPPS), Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Psychiatric Pharmacist (BCPP), Board Certified Sterile Compounding Pharmacist (BCSCP), or Board Certified Transplant Pharmacist (BCTXP).
