Job Summary:The Licensed Practical Nurse/Licensed Vocational Nurse will work under the direction of the CBOC Clinic Manager, PACT Provider, and STGi Program Manager to provide a wide range of clinical and administrative nursing care. This includes assessing patient conditions, administering medications, and performing various nursing procedures.
Responsibilities:- Assess patient conditions and develop individualized care plans
- Administer medications and perform various nursing procedures
- Work with the PACT team to check-in/check-out patients and participate in staff meetings and huddles
- Observe patients and report adverse reactions to medications, treatment, or physical/mental condition
- Perform cardiopulmonary resuscitation (CPR) and assist during respiratory and cardiac arrest procedures
- Document patient information in medical records and VISTA
- Assist in maintaining the VISTA clinic reminder tracking system and completing required reports
- Provide health education and materials to patients and their families
- Participate in the Performance Improvement Program and ensure compliance with federal, state, and local regulations
- Minimum of two (2) years of current clinical experience working in a similar environment
- Knowledge of basic methods and procedures for coordinating and scheduling clinical care
- Thorough knowledge of the variations in anatomy and practical nursing principles
- Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations
- Ability to follow orders and work under close supervision while demonstrating initiative and independence
The Licensed Practical Nurse/Licensed Vocational Nurse will work in a typical interior/office work environment with moderate physical activity. The position requires handling average-weight objects up to 25 pounds and standing/walking for more than four (4) hours per day.
