Licensed Practical Nurse Team Leader

The Licensed Practical Nurse Team Leader plays a vital role in providing exceptional care to our residents at Mercy Nursing Facility SNF Post Acute. As a key member of our team, you will administer nursing care to designated residents under the direction of a registered nurse, while also coordinating and supporting the activities of certified nurse assistants.
Responsibilities- Administer nursing care to designated residents under the direction of a registered nurse
- Coordinate and support the activities of certified nurse assistants
- Participate in the direct care of residents, following the specific approaches as stated in the plan of care
- Ensure the highest level of care and service to our residents
- High School diploma or GED
- Graduate from an accredited school of practical nursing
- Currently registered in New York State as a Licensed Practical Nurse
- BLS validation and re-validation in accordance to AHA standards
- Long term care experience preferred
