Casa Real is seeking a highly skilled and experienced Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in a Skilled Nursing role. This position requires a strong background in providing high-quality patient care in a fast-paced environment.
Responsibilities:- Provide direct patient care to residents in a Skilled Nursing facility, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments of patients' physical and emotional needs, and document findings in a clear and concise manner.
- Communicate effectively with patients, families, and healthcare team members to ensure a positive and supportive care environment.
- Current licensure as a LVN or LPN in the state of New Mexico.
- At least 2 years of recent experience in Skilled Nursing, with a focus on patient care and assessment.
- AHA BLS certification.
- Additional certifications related to Skilled Nursing, such as CNA or CMA.
