Job Title: Physical Therapist - Home Health - Oscoda County

We are seeking a skilled Physical Therapist to join our team at The Care Team Home Health & Hospice in Oscoda County. As a Physical Therapist, you will provide high-quality care to patients in their homes, helping them to recover from illness or injury and achieve their full potential.

Job Summary:

The Physical Therapist will work with patients to develop and implement individualized treatment plans, using a variety of techniques and modalities to promote healing and improve function. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Conduct initial and ongoing assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement individualized treatment plans in consultation with physicians and other healthcare professionals.
  • Provide skilled physical therapy services to patients in their homes, including exercises, modalities, and education.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in interdisciplinary team meetings and contribute to the development of patient care plans.
  • Stay up-to-date with the latest research and best practices in physical therapy, and apply this knowledge to improve patient outcomes.
Requirements:
  • Current licensure as a Physical Therapist in the State of Michigan.
  • Minimum of one year of experience in home health physical therapy.
  • Strong background in physical therapy, including knowledge of anatomy, physiology, and biomechanics.
  • Excellent communication and interpersonal skills.
  • Ability to work independently in a fast-paced environment.
  • Valid state driver's license and reliable transportation.
