Duties and Responsibilities- Deliver high-quality, patient-centered care to clients, focusing on their unique needs and preferences.
- Collaborate with interdisciplinary teams to develop and implement effective care plans, promoting social skills, boundaries, and daily living skills.
- Ensure accurate and timely documentation, including case notes and Medicaid records.
- Participate in treatment team meetings, contributing to the planning and implementation of recreational, educational, and vocational activities.
- Develop positive relationships with clients, motivating and encouraging them to complete required treatment activities.
- Provide crisis intervention when necessary, ensuring a safe and supportive environment for all individuals.
- Cultivate strong partnerships with community professionals, including NSHC staff and personnel from other agencies.
- Bachelor's degree or equivalent in a relevant field, such as healthcare or social work.
- High school diploma or equivalent required; some college coursework preferred.
- Current certifications in BLS/CPR, First Aid, and Assistance with Self-Administration of Medication.
- Strong interpersonal and communication skills, with the ability to build rapport with clients and colleagues.
- Excellent problem-solving and critical thinking skills, with the ability to adapt to changing situations.
