Job Summary

Freeman Health Partners is seeking a highly skilled Admissions Licensed Practical Nurse (LPN) to join our team. As an Admissions LPN, you will play a critical role in supporting the provision of patient care, including recording vital signs, discussing chief health concerns, performing tests, administering injections, and assisting with procedures and documentation.

Key Responsibilities:

  • Measure and record patient's vital signs, including blood pressure and pulse.
  • Discuss chief health concerns with patients and record them.
  • Obtain orders from the Medical Director for medication administration.
  • Administer and/or supervise the administration of prescribed medications.
  • Perform tests as required for patient care.
  • Keep medical records current.
  • Provide wound care as needed.
  • Prepare and administer prescribed medication.
  • Report any adverse reactions to medication to the supervisor and note in medical records.
  • Ensure electronic medical records are up-to-date.
  • Record all care information concisely, accurately, and completely in a timely manner.
  • Perform other position-related duties as assigned.

Admissions Responsibilities:

  • Review medical records of potential clients to ensure they meet program criteria.
  • Send records for approval from the Medical Director.
  • Medically assess clients upon arrival and complete initial nursing assessment.
  • Verify medications match prescriptions on containers.
  • Separate narcotic medications and communicate with transportation and/or secure medications in the Med Room.

Requirements:

  • Minimum of one year of technical training.
  • Minimum of one year of related work experience.
  • Current license in good standing with the State of Tennessee Licensing Board.
  • Current certification in First Aid/CPR, AED, and Bloodborne Pathogen training.
  • Knowledge of Medical Terminology.
  • Interpersonal/human relations skills.
  • Ability to maintain confidentiality.
  • Ability to file and retrieve information in EHR.
  • Ability to adhere to established departmental policies and procedures.

Work Schedule:

Monday-Friday, 1p-9:30p.
