Master Social Worker Ambulatory Care Specialist

Job Summary

This position is responsible for providing social work services to patients and families in an ambulatory care setting. The ideal candidate will have a Master's degree in Social Work and be licensed as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).

Key Responsibilities
  • Assess patients' functional and psychological needs, and develop plans to address these needs.
  • Collaborate with the healthcare team to develop and implement discharge plans.
  • Provide advocacy, assistance, support, counseling, and crisis intervention to patients and families.
  • Maintain knowledge of community resources and assist patients in accessing these resources.
  • Document all interventions in the patient's medical record.
Requirements
  • Master's degree in Social Work or related field.
  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
  • Basic Life Support (BLS) certification.
  • 3 years of acute care hospital experience.
Work Environment

This position is based in an ambulatory care setting, and the ideal candidate will have experience working in a similar environment.
