Pharmacy Services - Medication Oversight

This role involves managing medication protocols for various clinics, including anticoagulation management and post-discharge support.
Candidates must possess a residency or equivalent experience in anticoagulation therapy, particularly in warfarin management.

Key Responsibilities:
- Ensure the correct selection, dosing, administration, and dispensing of medication orders.
- Review and evaluate medication orders to identify, prevent, and resolve potential drug-related issues. Recommend formulary alternatives when necessary.
- Prepare and dispense medications accurately for patient delivery.
- Provide drug information and education to healthcare professionals and patients.
- Collaborate with physicians and other healthcare providers to facilitate optimal patient care.
- Supervise technical staff involved in the preparation and dispensing of medications and supplies.
- Participate in academic activities, including serving as a preceptor for pharmacy students.
- Provide patient counseling as needed.
Licensure / Certification / Registration:
Must obtain pharmacist licensure from the Indiana Board of Pharmacy within 90 days of hire or transfer.
Education:
A Bachelor's or advanced degree in Pharmacy is required.
Additional Preferences:
Completion of a residency or equivalent experience is preferred.
