Job Summary

We are seeking a skilled Registered Nurse to join our Weekend On-Call team. As a Weekend RN, you will provide on-call services for personal, physical, and emotional needs of patients and their families. You will be responsible for evaluating and responding to patient needs via telephone contact, home visits, and/or facility visits.

Key Responsibilities
  • Provide on-call services for patients and families, including home visits and telephone contact.
  • Evaluate and respond to patient needs, including physical and emotional assessments.
  • Develop and maintain written Hospice Care Plans for each patient.
  • Maintain accurate and up-to-date medical records and other records pertaining to patient care.
  • Participate in weekly patient care conferences and communicate with team members regarding patient status.
  • Collaborate with Interdisciplinary Group members to ensure best care for patients according to the Hospice concept.
  • Develop Hospice Aide plans of care based on patient personal care needs.
  • Notify Team Leader of changes in patient status, including hospitalizations and discharges.
  • Assist in creating a climate that promotes growth and harmonious relationships within the Hospice staff.
  • Promote a positive image of the Hospice concept throughout the community.
Requirements
  • Minimum of one year of clinical nursing experience, preferably in Hospice or medical/surgical settings.
  • Current RN license in the State of Florida or Alabama, as applicable.
  • Reliable transportation, valid driver's license, and current automobile liability insurance.
  • CPR certification.
Special Demands/Physical Requirements

This position requires medium to heavy lifting, frequent travel, and mental demands due to assessing and evaluating patient care.
