Job Summary

We are seeking a highly skilled and compassionate Addiction Counselor to join our team at Kings County Hospital Center. As a key member of our Behavioral Health/SUD Treatment Programs, you will play a vital role in providing high-quality care to individuals struggling with substance use disorders.

Key Responsibilities

  • Conduct initial screenings and assessments of patients, performing ongoing toxicology screenings to gather pertinent information about SUD and Co-Occurring Disorders.
  • Develop psycho-social histories, formulate diagnostic impressions, and prepare accurate and timely screening, intake, and assessment reports.
  • Facilitate and/or co-facilitate individual, group, and family counseling, providing psychoeducation and conducting skills training.
  • Make decisions and maintain required records regarding appropriate Level of Care, using patient criteria tools and clinical judgment to ensure patients receive the most clinically appropriate level of care.
  • Develop and continually monitor and update treatment/recovery plans in collaboration with patients and the clinical treatment team, establishing short and long-term treatment goals, objectives, and interventions.
  • Coordinate and provide case and care management services and activities, making referrals and linkages as needed.
  • Initiate and finalize discharge and transition planning, collaborating with patients and the clinical treatment team to ensure a smooth transition.
  • Maintain timely, accurate, and appropriate documentation in compliance with all applicable standards to ensure proper reimbursement.
  • Provide orientation to new patients, families, and significant others, discussing financial responsibility and providing money management assistance as needed.
  • Participate in relevant staff meetings, case conferences, and trainings, contributing to special projects and internal/external quality assurance/performance improvement activities.
  • Participate in community outreach activities on and off campus, promoting the mission and values of NYC Health + Hospitals.

Requirements

  • Valid CASAC - Level 1 Certificate issued by OASAS, or equivalent qualification.
  • One year of full-time, progressively responsible experience performing SUD assessment, treatment planning, individual and group counseling, case and care management, and other functions related to SUD prevention and treatment in a SUD treatment program.
  • Satisfactory equivalent of education, training, and experience, with a CASAC Level 1 Certificate issued by OASAS or meeting the definition of a QHP as described in "2" above.
