Key Responsibilities:- Provide medical care to patients in their homes, including assessments, treatments, and follow-up visits.
- Collaborate with our interdisciplinary team, including nurses, therapists, and social workers, to ensure comprehensive care coordination.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Participate in ongoing education and training to stay current on best practices in hospice care.
- Current licensure to practice medicine in the state of employment.
- Board certification in a relevant medical specialty.
- Minimum of 2 years of experience in hospice care or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
