Responsibilities- Rounding and managing clinical care of residents in skilled nursing facilities
- Prescribing medications and treatments as needed
- Providing high-quality, patient-centered care to residents
- Timely documentation of all patient care and encounters
- Availability for phone and secure messaging consultations with facility nursing staff
- Current licensure as a Nurse Practitioner
- Experience in skilled nursing facilities or post-acute care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Reliable transportation
