Clinical Pharmacist PRN

We are seeking a highly skilled Clinical Pharmacist to join our team at Children's Health. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our inpatient pharmacy.
Key Responsibilities- Review and verify medication orders to ensure accuracy and appropriateness.
- Assess the effectiveness, safety, and affordability of patient medications, and work collaboratively with the healthcare team to develop and optimize patient medication therapy.
- Provide drug information and education to physicians, nurses, and patients/families, and monitor profiles for drug interactions and duplication of therapy.
- Participate in quality improvement activities, including floor inspections, medication use evaluations, and documentation of interventions.
- Support the development and implementation of Epic order entry and set build, and participate in hospital committee meetings as needed.
- Identify and understand issues, problems, and opportunities, and use effective approaches to develop solutions.
- Deal effectively with others in an antagonistic situation, and use appropriate interpersonal styles and methods to reduce tension or conflict.
- Accomplish tasks by considering all areas involved, and show concern for all aspects of the job.
- Set high standards of performance for self and others, and assume responsibility and accountability for successfully completing assignments or tasks.
- At least 1 year of experience in hospital pharmacy practice.
- At least 5 years of experience in any other pharmacy practice; management experience preferred.
- Four-year Degree (B.S. or Pharm.D) from an accredited school of pharmacy required.
- Current Texas Pharmacist license, required.
- BLS Certification, required.
- IV certification, required.
- Board of Pharmacy Specialties certification, preferred.
