About the Role

The Licensed Practical Nurse (LPN) is a vital member of our Assisted Living team, responsible for providing high-quality clinical care and services to residents. This role requires a strong commitment to delivering exceptional patient care, while also promoting independence and overall well-being.

Key Responsibilities
  • Provide direct care to residents, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team.
  • Conduct regular assessments and evaluations to ensure residents' needs are met.
  • Collaborate with other healthcare professionals to ensure seamless care transitions.
  • Participate in quality improvement initiatives to enhance resident outcomes.
Requirements
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of Minnesota.
  • Current CPR certification.
  • Experience in gerontological nursing, rehabilitative, and restorative clinical practices.
  • Demonstrated ability to read, write, speak, and understand the English language.
