Capitol Home Health - Clinical/Field is seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes.
Job Summary
The successful candidate will be responsible for evaluating patient needs, developing treatment plans, and delivering therapy services in a home care environment. This role requires strong communication and organizational skills, as well as the ability to work independently and as part of a team.
Key Responsibilities
- Evaluate patient needs and develop individualized treatment plans
- Deliver physical therapy services in a home care setting
- Collaborate with physicians, case managers, and other healthcare professionals to ensure comprehensive care
- Document patient progress and maintain accurate records
Requirements
- Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy
- Current and unencumbered licensure to practice as a Physical Therapist in the state of employment
- At least two years of experience as a Physical Therapist, preferably in a home health or community setting
- Excellent communication and organizational skills
- Access to reliable transportation
