The Physical Therapist (PT, DPT, RPT) in our home health setting is tasked with assessing and evaluating patient care requirements related to mobility, endurance, pain, and other health conditions. This assessment informs the development of a tailored treatment plan, which includes interventions aimed at enhancing the patient's overall well-being and monitoring their progress.
- Conducts evaluations of the patient's physical therapy needs and functional status, assisting in the formulation of the therapy care plan.
- Monitors and communicates the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
- Administers treatments to alleviate pain, restore function, and maintain optimal physical capabilities following established standards of practice.
- Educates and informs all members of the healthcare team, the patient, and their family or caregivers about the care plan, functional limitations, and progress towards rehabilitation goals.
Licensure Requirements:
- Active Physical Therapy license in the state of practice.
- Current CPR certification is mandatory.
- Possession of a valid driver's license and vehicle insurance, along with access to a reliable vehicle.
