Job Title: Physical Therapist - SNF

Naamans Creek Country Manor - Powerback is seeking an experienced Physical Therapist to join our team in a SNF setting. As a Physical Therapist, you will be responsible for managing a treatment plan to promote a positive outcome for patients with diverse diagnoses.

Job Summary:

We are looking for a Physical Therapist who is passionate about delivering high-quality patient care and has experience working in a SNF setting. The ideal candidate will have a strong understanding of physical therapy principles and practices, as well as excellent communication and interpersonal skills.

Responsibilities:
  • Develop and implement individualized treatment plans for patients with diverse diagnoses
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Provide physical therapy interventions, including exercises, modalities, and education
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Document patient progress and outcomes
Requirements:
  • Current BLS (AHA Preferred)
  • Active State License
  • Supervisory Professional References
  • Must complete Drug Screen and Background Screen
