Job Title: Per Diem Home Health RN

Join our team of dedicated healthcare professionals at Lancaster General Health as a Per Diem Home Health RN. As a member of our home health team, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Job Summary:

We are seeking a compassionate and skilled RN to join our home health team. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct regular home visits to monitor patient progress, provide education, and address any concerns or questions.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
Requirements:
  • Current RN license in the Commonwealth of Pennsylvania.
  • CPR Certification - American Heart Association (AHA) Basic Life Support.
  • Valid PA Driver's license and auto insurance.
  • Minimum one year clinical experience required.
  • Preferred qualifications: one year of Home Health experience.
