Responsibilities
  • Conduct comprehensive diagnostic assessments to identify client needs and develop personalized treatment plans.
  • Provide patient visits as scheduled, ensuring timely and effective care.
  • Ensure treatment plans are up-to-date and reflect client progress.
  • Document client progress and treatment outcomes in comprehensive and succinct progress notes.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Collaborate with case managers and psychiatrists to ensure comprehensive care.
Requirements
  • Bachelor's degree in Social Work or a related field, with one year of therapy experience, or a Master's degree in Social Work or Professional Counseling.
  • Current Social Work/Professional Counseling License or equivalent (LSW, LISW, LISW-S, LPC, LPCC, LPCC-S).
  • Experience in therapy and counseling, preferably in a home setting.
  • Valid driver's license, car insurance, and reliable transportation.
  • Willingness to travel throughout assigned service areas.
