Job Title: Assisted Living RN Manager

Grace Prairie Pointe is seeking a highly skilled and compassionate Assisted Living RN Manager to join our team. As a key member of our leadership team, you will be responsible for providing exceptional care and support to our residents.

Responsibilities:
  • Conduct thorough assessments and develop individualized care plans to meet the unique needs of each resident.
  • Oversee the admission process, ensuring a smooth transition for new residents and their families.
  • Monitor and manage medication administration, including self-medicators and psychotropic medications, to ensure resident safety and well-being.
  • Collaborate with healthcare providers to ensure accurate and timely documentation of resident health status.
  • Develop and implement effective care plans, including diabetic care and medication management.
  • Supervise and train staff to ensure they are equipped to provide high-quality care and support to residents.
  • Monitor and maintain accurate records, including vital signs, lab results, and medication administration.
  • Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance resident care and satisfaction.
Requirements:
  • Valid Wisconsin Registered Nurse license.
  • Three to five years of nursing experience, with a minimum of two years in long-term care.
  • Excellent leadership and communication skills, with the ability to motivate and inspire staff.
  • Supervisory experience preferred, with a proven track record of success in a leadership role.
  • BSN preferred, but not required.
