Department Overview

We're passionate about understanding what matters to people to ensure the delivery of Real-Person Care. Our Care Solutions, Analytics, Implementations, Clinical and Business Operations, and Learning and Development teams work together to address real-life healthcare needs.

Job Summary

We're seeking a skilled Next Site of Care Coordinator to work with our site Case Manager lead to identify patients with potential Transitional Care Unit (TCU) discharge disposition to TCU. This role will facilitate next site of care discussions, work with Unit Case Management, and communicate with Post-Acute Care Transition of Care Managers.

Key Responsibilities
  • Identify patients with potential TCU discharge disposition to TCU
  • Facilitate next site of care discussions with Unit Case Management
  • Communicate with Post-Acute Care Transition of Care Managers
  • Introduce Ambulatory Care Management Services to patients
  • Participate in multidisciplinary rounds
  • Develop and drive action plans for performance improvement
  • Escalate cases to Physician Advisors when disposition disconnect occurs
Requirements
  • Bachelor's degree in a healthcare-related field
  • 5 years of operational or clinical experience in Inpatient Care Management or Discharge Planning services
  • Registered Nurse licensed to practice in Minnesota
  • Strong leadership skills and ability to prioritize multiple asks and deadlines
  • Excellent communication and problem-solving skills
