Job Summary:
The Case Manager (CM) role is responsible for ensuring the delivery of rehabilitative care that meets the needs of each patient. The CM coordinates efforts within the Rehabilitation team and collaborates with directors to ensure departmental functions meet high standards.
Key Responsibilities:
- You will provide orientation to patients/families/caregivers, outlining your role, the rehabilitation philosophy, continued stay/discharge criteria, Medicare/insurance benefits, grievance procedures, treatment plan processes, and rights/responsibilities.
- You will act as coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans/goals/status through Family Conferences and other interactions.
- You will implement the patient's treatment plan, supporting their strengths, abilities, needs, and preferences, and facilitate involvement throughout the rehabilitation process.
- You will document Discharge Planning Evaluations and psychosocial assessments, communicating social/financial/discharge needs/preferences of the patient/family/caregiver.
Requirements:
- Current State of Alaska Registered Nurse License.
- American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) certification.
- Associates Degree in Nursing required, BSN preferred.
- One year of Rehab or Med/Surg experience preferred, Experience in Utilization Review, Discharge planning, or Case Management preferred.
