Home Health Nurse Opportunity

We are seeking a skilled and compassionate Registered Nurse to join our team in the Fort Worth area. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes.

Responsibilities:
  • Conduct thorough nursing assessments and develop personalized care plans in collaboration with patients, families, and healthcare teams.
  • Evaluate patient progress, adjust care plans as needed, and ensure timely communication with healthcare teams.
  • Provide skilled nursing services, including medication administration, wound care, and therapeutic treatments, as ordered and indicated.
  • Deliver patient education and health counseling on disease management, self-care techniques, and prevention strategies.
  • Participate in health promotion and disease prevention activities, and coordinate services with the Clinical Team Manager and Clinical Team Coordinator.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current unrestricted Texas RN license or compact license.
  • BLS certification or ability to obtain within 30 days of employment.
  • At least 1 year of experience in home health care or a related environment.
  • Knowledge of Microsoft Office applications, including Outlook, Word, and Excel.
  • Ability to read, analyze, and interpret medical records and health plan documents.
  • Ability to lift and transfer patients.
  • Access to reliable transportation for client and patient visits.
Preferred Qualification:
  • Bilingual (Spanish) skills.
