Healthcare Specialist

As a registered nurse at BAYADA Home Health Care, you will have the opportunity to make a positive impact on the lives of our clients. With a strong focus on delivering high-quality patient care, we are seeking an experienced and compassionate nurse to join our team.

• Clinical Care: Develop and implement personalized care plans tailored to each client's unique needs, working closely with healthcare teams to ensure seamless transitions and optimal outcomes.
• Home Visits: Conduct regular home visits to assess client conditions, provide medical interventions, and educate clients on self-care techniques to promote independence and well-being.
• Case Management: Coordinate with interdisciplinary teams to identify resource needs, develop strategies for addressing social determinants of health, and advocate for clients' interests.
• Communication: Maintain open and transparent communication with clients, families, and healthcare providers to ensure that all parties are informed and engaged in the care process.

• A current license as a Registered Nurse in Vermont is required.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience is preferred.
• Wound Care Certified is a plus.
• Prior home care experience is strongly recommended but not mandatory.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) is beneficial.

• We offer a competitive salary range of $80,000 - $110,000 per annum, depending on experience and qualifications.
