Adult Family Living Program Evaluator

Eminence Home Care is in search of a proficient Adult Family Living Program Evaluator to become a vital part of our organization.
This role is essential for the effective screening and onboarding of new clients into our Adult Family Living initiative.
The ideal candidate will demonstrate exceptional intake and evaluation capabilities, with a thorough understanding of assessing functional requirements, home conditions, and safety measures.
The evaluator will also be responsible for confirming fundamental funding sources for care. This position reports directly to the Director and is supported by a dedicated office team for task assignments. Flexibility in scheduling and a readiness to travel within the designated area are crucial.
Your Responsibilities:
1. Conduct thorough evaluations of prospective Adult Family Living clients to ascertain their functional requirements, home environment appropriateness, and safety considerations.
2. Confirm basic funding sources for clients' care.
3. Execute intake procedures efficiently and accurately to facilitate seamless client onboarding.
4. Collaborate with community partners, including Skilled Nursing Facilities (SNFs), networking associates, and Area Agencies on Aging, to enhance referrals and new client intake.
5. Maintain comprehensive records of evaluations and client interactions.
6. Provide regular updates and reports to the Director regarding client needs and evaluation results.
Qualifications:
• Bachelor's degree preferred in social work or a related discipline.
• 1-2 years of experience in home care, focusing on intake, assessment, and home visits.
• Familiarity with the CT Home Care Program and/or Adult Family Living Program.
• Knowledge of DSS Medicaid or Private Sector Home Care.
