About the Role

We are seeking a skilled and compassionate Home Health RN to join our team at Providence Home and Community Care. As a Home Health RN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.
  • Administer medications and treatments as prescribed by physicians, and monitor patients' responses to these interventions.
  • Provide education and support to patients and their families on disease management, medication use, and other health-related topics.
  • Document patient care and progress in a timely and accurate manner, and communicate effectively with other healthcare professionals.
Requirements
  • Graduate of an accredited school of nursing.
  • Current California Registered Nurse License.
  • Basic Life Support (BLS) certification.
  • At least one year of acute care nursing experience.
Preferred Qualifications
  • Coursework or training in infusion nursing.
  • Experience in home health or home infusion.
  • Prior sales or marketing experience.
